With this information On august 20, 2021 at 5:30 am patient XYZ was found in the parking lot of a hotel in Arkansas Valley unresponsive. The patient has a history of depression, substance abuse, essential hypertension, chronic pain, asthma, coronary artery disease, CHF, and chronic kidney disease.Currently the patient is post op with the ventriculostomy and burr hole in place. The incision site in intact and there is no sign of infection. The patient is on a ventilator and is still sedated. She is not breathing on her own, the ventilator has full control of her repertory rate. The patient does not respond to verbal commands, is not oriented, and withdraws from pain stimulus.
