Two of the most important moral principles or values in health-care are (1) respect for the patient’s autonomy and (2) promotion of the patient’s well-being.  Well-being is the concept of a person’s welfare, what’s good for a person, or what’s in a person’s interests; what most promotes a person’s well-being is what’s best for a person, or what’s in a person’s best interests.  Autonomy, on the other hand, is what a person wants, chooses, decides, or intends for their own life, or what a person values or desires for themselves. Importantly, autonomy and well-being can come apart, certainly for multiple people (what one person wants might make another person worse-off), but even for a single person (what a person chooses might not be what’s in his or her own best interests).  This can be the case if a person’s values aren’t focused on his or her own welfare (for example, if the person primarily values her religious obligations, or something else that the person deems worthy of self-sacrifice, e.g. one’s family).  Autonomy and well-being can also diverge even if a person is trying to choose what’s best for herself, but fails to do so (for example, someone might choose to smoke because she mistakenly believes that the pleasure she gets from smoking outweighs the pain, suffering, and early death that she is risking).In health-care, conflicts between autonomy and well-being arise all the time: a patient might make a decision about his treatment that isn’t what’s best for him, or a decision that’s not best for someone else (this is often the case with the distribution of scarce resources in hospitals).  So, here is a very general and open-ended question: Which is more important in health-care, autonomy or well-being?  Which is the primary goal of nursing practice?  Does it depend on the situation?  If so, how, i.e. what kinds of situations call for an emphasis on autonomy, and which for a focus on well-being?   Health Science Science Nursing GPNG 526 Share QuestionEmailCopy link Comments (0)
