Using a provider example from Illinois or Iowa

Option #1: Transitions of Care, Letter to Provider

As you read this week, keep in mind that healthcare advocacy plays an important role in supporting the needs of the population, and that the Patient Protection and Affordable Care Act (PPACA) offers many opportunities for healthcare advocacy.

The Community-based Care Transitions Program (CCTP) is an example of a program that focuses on the development of models that are designed to improve patient care transitions and to reduce hospital readmissions. See the CTTP Fact Sheet for a brief overview of the Community-Based Care Transitions Program and information on how consumers can advocate in this area. [For additional information, you may visit the Centers for Medicare and Medicaid Services (CMS) at https://innovation.cms.gov/initiatives/CCTP/.]

Directions:

  1. After reviewing the fact sheet and visiting the CMS website, compose a two- to three-page letter (this length does not include the required title and reference pages) to a geriatrician, a community-based health organization, or a hospital in your community that encourages the development of a proposal for a Community-based Care Transitions Program (CCTP) in the community.
  2. The letter should include the name and address of the provider, the date, an introductory paragraph, a body, a concluding paragraph, and a signature block.
  3. Be sure to include some of the benefits of the CCTP to the community, to Medicare beneficiaries, and to the healthcare system.
  4. Include in-text citations and cite them on the reference page. Use two to three credible sources. The CSU-Global Library is a good place to find these sources.
  5. Format your entire paper according to the CSU-Global Guide to Writing and APA.

Option #2 : Transitions of Care, Talking Points Outline

As you read this week, keep in mind that healthcare advocacy plays an important role in supporting the needs of the population, and that the Patient Protection and Affordable Care Act (PPACA) offers many opportunities for healthcare advocacy.

The Community-based Care Transitions Program (CCTP) is an example of a program that focuses on the development of models that are designed to improve patient-care transitions and reduce hospital readmissions.

Directions:

After reviewing the fact sheet and visiting the CMS website, develop a two- to three-page outline (this does not include the required title and reference pages) of your plan to convince providers in your area to submit a proposal for funding of a Community-based Care Transitions Program (CCTP) in your community. You will take this outline with you when you meet with the different providers and use it as talking points.

Your document should include the following.

  • The names and addresses of three potential providers (geriatrics provider, community-based health organization, and hospital) in your community who you believe might be interested in the development of a proposal for a Community-based Care Transitions Program (CCTP). Place this information on one page.
  • An outline of talking points that you will cover during your meetings with the different providers. Place this information on one to two pages.
  • The outline should include the following five basic elements with two to three sentences written about each.

I. What is the Community-based Care Transitions Program (CCTP)?
II. How will the CCTP improve transitions of care for Medicare beneficiaries?
III. How will the CCTP reduce hospital readmissions for high risk Medicare beneficiaries?
IV. How will the CCTP improve quality of care?
V. How will the CCTP reduce costs for the Medicare Program?

  • Include in-text citations and reference these on the reference page. Use two to three credible sources. The CSU-Global Library is a good place to find these sources.
  • Your assignment must be written according to the CSU-Global Guide to Writing and APA.
