Question The patient had an implantable pacing cardioverter-defibrillator (ICD)placed 6 months ago. The patient now has a malfunction of this device. After adequate anesthesia, an incision was made and the electrodes were removed from the generator. There was a screw problem with the generator that was producing a noise. The leads were tested and found to be working satisfactorily. The original implantable defibrillator pulse generator was removed and then replaced with an implantable defibrillator pulse generator. Both generators were a dual lead system. The ICD was placed back in the pocket, and the wound was closed after irrigation with 3-0 and 4-0 Vicryl.
