Question Case studyLisa Gomez is a 57 yo female. Pt arrived at Ed via EMS from lived in car due to bizarre/ dangerous behavior. Per pt’s daughter, she had picked pt today to the welfare office to obtain Medicaid renewal forms. She reports that on the way went home, pt “had an episode” in which she became upset asking daughter “who you are, where are you taking me”. Pt grabbed the car wheel, almost causing accident and try to pull her daughter out of the car. She called police at this time who were able to help pt go with EMS to hospital. She states that pt will “kill herself and someone else if she is let out”. At the admission psychiatric interview, pt presents as distressed, but cooperative with depressed mood and tearful affect. Pt’s thought process is disorganized and tangential, required constant redirection. Pt’s thought content is exhibits paranoid delusions of her adult children being in danger of “being tortured and/ or killed”. Pt reports A/V/H in the form of voices telling her that her child are in danger and seeing “figures flying and dancing around” or “I wish someone would blow my head off” or when I lost my kids, way back, I took a bottle of aspirin. it made me throw up. Per daughter last week at dinner pt took a knife and help it up to her chest stating “I can’t take it anymore the voice won’t go away. Pt states that she “doesn’t trust anyone and doesn’t know what going on” and “hasn’t taken any medications for the past 2 months”. Pt refuses drug abuse but the states ” if I can’t get medications, I have to medicate myself”.
