Jane Rosen, RN, BSN, has recently been promoted to clinical team… Jane Rosen, RN, BSN, has recently been promoted to clinical team coordinator in the medical geriatric diabetic unit. While she has been a nurse for only 18 months, Jane has demonstrated strong leadership potential and an enthusiasm for problem solving and innovation. Jane’s manager soon approaches her with the first opportunity to exercise her new leadership role in a major change initiative.Nursing leadership has been considering how clinical practice could be altered for at-risk populations in a way that could engage them earlier and manage their chronic conditions by addressing the continuum of care and reducing the incidence of late-stage, late-engagement, high-intensity hospital care. The geriatric, diabetic patient population has been one of those patient groups selected for testing out new models of care. The proposed model would emphasize earlier engagement in patient care management and the longer-term continuum of care service structure for delivering care to this population of patients.Jane has been asked by her manager to put together a small team of nursing colleagues to plan ways in which care could be designed and delivered differently for the geriatric diabetic population so as to directly effect a reduction in hospital inpatient admissions, a higher level of quality of service, and a stronger positive impact on the lives of geriatric diabetic patients.
