The patient states she does not take diabetic medicine because it made her feel gassy. The patient states she stopped the medication without talking to her doctor. The patient also has an HGBA1c of 8.7 and verbalizes she has her blood sugar under control though she does not check her CBG regularly. This patient does not exercise because she is on her feet all day at work, and she eats a lot of bread products daily. It is quite possible her foot is not healing because of her uncontrolled diabetes. With this data, Tina clearly needs education for a healthier lifestyle.In order to maximize her participation and control of her diabetes, I would provide diabetic interactive education for Tina especially focusing on nutrition and how to count carbohydrates including using a food and carbohydrate log. Educating her also on the importance of monitoring her feet and explaining that diabetics are at much higher risk of infection if feet or wounds are neglected. Because information can be overwhelming in the hospital, I would provide handouts for her to reference and the American Diabetic Association website information and arrange for her to be followed closely by her provider. I would explain the importance of monitoring her CBG regularly and HGA1c to use as a gauge of how well her diabetes is being controlled and also for positive reinforcement that reflects her lifestyle changes as the HGBA1c goes down. I would also explain how the HGBA1c measures the blood sugar over 3 months so she really understood the effect of it going down.When planning and implementing interventions with patients, the critical thinking dimensions of contextual perspective, flexibility, and open-mindedness are involved as well as loads of perseverance and creativity are necessary to meet desired outcomes (Rubenfeld & Scheffer, 2014). When teaching a patient, it is helpful to assess the baseline of the patient’s knowledge to identify topics that need to be addressed first. Ms. Jones’ statement of avoiding sweets instead of pricking her fingers all the time is a good start on assessing her baseline knowledge about diabetes. The age of the patient is also an important consideration when providing education to patients. In the study by Dehkori & Abdoli (2017), it was highlighted that interactive learning is the preferred and most effective method of lecturing for young adults with diabetes. Given that Tina is only 28 years old, interactive teaching and the use of technology are beneficial for her improved learning outcome.
