The student nurse reports to you that Mei fell during the night at approximately 0400 and Mei did not call for help when she needs to get up but seems to be fine this morning. The student nurse states that Mei fell as she was getting up to use the commode and Mei denies loss of consciousness. Her daughter states that the fall was not witnessed, but she was awakened at night to assist her mother after the fall. Mei was placed back in bed by her daughter who says that her mom has been unsteady in the past 2 days. Mei has a 3 cm abrasion on her leg. You instruct the student nurse to monitor pain and mobility issues. You tell the student nurse that you will be making a home visit this morning for an assessment of the client’s condition and situation. You visit the home that morning at 1100. You observe a 3 cm abrasion on the left leg. Mei is moving all limbs and has no complaints of pain at this time. BP 130/86, T 37.1 C (t), P94, R22. PEARL. You measure Mei’s blood glucose level. You apply a clean dressing to the wound. Her daughter states that the fall was not witnessed, but she assisted her mother after the fall because the nurse was not there. Also, Mei tells you that she called the student nurse many times before she attempted to get up, but with no reply.
