This website has been completely updated in the past 6 months. You can do as many of the self-tests as you’d like, but I want you to do the following tests and report the results by answering the questions. I recommend you copy the questions below and paste to a Word doc and then answer the questions on the Word doc as you go through each self-assessment. At the completion, you can upload the Word doc to this assignment.

  1. Diabetes Risk (go down the page to find it). This is such a common chronic disease and everyone needs to self-assess on this one. Answer the following 4 questions:
    • What is your risk?
    • What recommendations were given to decrease your risk level?
    • Where you given screening information, and if so, what was it?

  2. Select either the Breast Cancer or the Prostate Cancer screening and answer the questions. These are the top most commonly occurring cancers for men and women. Answer the following 4 questions:
    • What is your risk?
    • What recommendations were given to decrease your risk level?
    • Where you given screening information, and if so, what was it?
    • What did it say in the “Keep up the good work” section?
    • Click on the Prevention Tips but you don’t need to answer any questions here. Just read it.
  3. Select the Lung Cancer option and answer all of the questions. Answer the following 4 questions:
    • What is your risk?
    • What recommendations were given to decrease your risk level?
    • Where you given screening information, and if so, what was it?
    • What did it say in the “Keep up the good work” section?
    • Click on the Prevention Tips but you don’t need to answer any questions here. Just read it.
  4. Select one more type of cancer that might run in your family or which holds an interest for you. Answer all of the questions until you get to the results. Answer the following 4 questions:
    • What is your risk?
    • What recommendations were given to decrease your risk level?
    • Where you given screening information, and if so, what was it?
    • What did it say in the “Keep up the good work” section?
    • Click on the Prevention Tips but you don’t need to answer any questions here. Just read it.
