Asta Bagdonas is a 52-year-old lady who was diagnosed with multiple… Asta Bagdonas is a 52-year-old lady who was diagnosed with multiple kidney stones 5 years ago. She suffers from diabetes mellitus type 2 (diagnosed in 2020). Asta was born in Lithuania and now lives in a small town in Buckinghamshire. She is married and has 2 daughters (25 and 28 years-old). On diagnosis of her diabetes Asta was started on Metformin 500 mg after breakfast and with a balanced diet she had well controlled blood glucose levels. Asta was experiencing digestive side effects from the Metformin, and her renal function was deteriorating, so the medical team decided to change her medication to Gliclazide 40 mg BD oral and insulin 5 UI Actrapid (if CBG>10 mmol/dL). Asta’s current medication is: Gliclazide 40 mg BD oral (breakfast and dinner) Insulin Actrapid 5 UI – PRN – SC – if CBG>10mmol/dL Paracetamol 1000 mg oral PRN – when experiencing renal colic pain. Tramadol Hydrochloride oral 50 mg PRN – when experiencing severe renal colic pain. Diazepam 5 mg oral – PRN – if experiencing insomnia or anxiety. Asta was driving to the work this morning when she started feeling dizzy, sweaty and lightheaded. She only had time to stop the car at the side of the road, she was seen by passers-by to be very drowsy, clammy and looked pale. They called an ambulance.
