The neonate has also undergone laboratory tests where WBC and hematocrit levels were determined to be elevated. The nurse assessed the neonate’s vital signs: Temp: 37.8C, HR: 165 bpm, RR: 64 bpm. The neonate’s skin was warm to touch, and was weak with reduced movement and reduced sucking prior to admission to ICU. The nurse also determined that decreased skin turgor was present and that the neonate’s urine output was decreased and more concentrated than normal. Due to the neonate being brought to the ICU, the mother is unable to provide breastfeeding continuously because of separation from the baby. NURSING DIAGNOSIS: Imbalanced Nutrition: less than body requirements related to interrupted breastfeeding due to continuously separation of mother from baby.
